We would like to thank you for taking part in our new process of accepting the required athletic participation paperwork for Jourdanton ISD. You will be able to complete all of the required forms except the medical history and physical.

Completing RankOne Forms

1. You must have the following information to complete the forms.
    1. Student's ID Number
    2. Insurance Group and Policy Number
    3. Contact information for emergency contact other than parents/guardians.
2. Go to this site.
3. Click on "Electronic Participation Forms" in the blue bar at the top of the page.
4. Select "Acknowledgment of Rules and Guidelines"
5. Fill out the form and click "submit"
6. Repeat step 3 or click "complete next form" on the submit page
7. Complete the "Emergency Contact and Consent to Treat" form and click submit
8. Repeat step 3 or click “complete next form” on the submit page.
9. Complete the “JISD Drug Testing Policy and Consent” and click submit.

Student-athletes may not participate in athletic activities (to include the athletic period) until all paperwork is complete and turned in to the Athletic Trainer.

Yearly Requirements

Physicals dated prior to May 1, 2019 will not be accepted for the 2019-2020 school year.
